New patient Registration
As a new patient to Elgin Pain Management, it is important that we have up-to-date information regarding your details and medical history. Please take a few moments to complete the following questions, which will be added to your records.
Brief pain inventory
Before every consultation and after every pain treatment we will ask you to complete the following, short questionnaire. It assesses the severity of your pain and how it impacts your daily functioning and will help us determine how effective your pain treatment is.
PAIN SELF EFFICACY QUESTIONNAIRE
The Pain Self Efficacy Questionnaire is designed to assess the confidence you have in performing activities whilst in pain.
POST PROCEDURE QUESTIONNAIRE
After every procedure, it is important that you complete the Post Procedure Questionnaire to give us an insight into how effective the treatment was.
PELVIC PAIN IMPACT QUESTIONNAIRE
The Pelvic Pain Impact questionnaire assesses the effect pelvic pain has on your quality of life. This questionnaire should be filled in prior to each consultation by those experiencing pelvic pain.
Quality of medicinal cannabis use survey
This survey is to be filled by all patients prescribed medicinal cannabis by Elgin pain management to assess the quality of effectiveness of this form of treatment. All information provided is confidential in nature
Referrals
To see one of our doctors and for all procedures you will require a current referral. A referral can be obtained from your general practitioner (valid for 12 months) or from another medical specialist (valid for 3 months). This referral is necessary for you to claim the Medicare rebate for our specialist services.
Your consultation
Your initial consult will take place in our Northcote practice. Whilst we can perform minor procedures in our rooms, more extensive procedures are carried out in a hospital setting.
If your pain management treatment requires more extensive treatment we will refer you to a public hospital pain clinic. Our doctors will generally perform this procedure.
Health insurance
Our medical doctors practice predominantly in the private sector and are happy to see both uninsured and insured patients. However, having private insurance will reduce the time you need to wait for any extensive procedures.
If you are uninsured or from overseas, you can choose to pay for your procedure in private hospitals attended by our doctors. We can help you obtain a quote from the hospital that you are going to.
Fees
Consultations and minor procedures
There will be out-of-pocket costs for your consultation or any procedures performed in the rooms. We will process a patient claim from medicare on your behalf for the consultation and procedure (if needed) on the day. There are reduced rates available for Pensioners and Health Care cardholders. Veteran’s Affairs Gold Card holders will be fully covered by the DVA.
EFTPOS, Electronic Transfer or Phone payments are preferred as no cash is held on premises.
Hospital procedures
For procedures carried out in hospital, our practitioners are all registered as a no-gap doctors with most major health funds. With most procedures, there will be a gap. These will be discussed with you prior to any treatment. Additional fees may arise if the services of an anaesthetist and radiographer are required.
